We are pleased and excited for you to sign up for a card with the Grafton-Midview Public Library!
You may fill out our online card application by clicking the link below.
You may also come in during normal business hours to apply for a card in person at the library.
We encourage all of our patrons to sign up for a card, either in person or online.
Please take note that adults need to have valid identification (see the following paragraph) to sign up for a card. Minors age 12-17 may sign up for a card without a parent or guardian. Minors under the age of 11 may sign up for a card with a parent or guardian.
To confirm all information provided, the following unexpired forms of identification are acceptable for adult applicants to bring to the library:
- Driver's License
- State ID
- Military ID
All new patrons are limited to 5 total checkouts at one time during the first 45 days after signing up for a card. Once that time has passed, the card is automatically updated to allow full access, as per our patron guidelines.
The limits of temporary status are described below:
- 5 total checkouts - combination of books, magazines, DVDs, CDs, Blu-Rays, and audiobooks.
- Limited to one (1) TV series at one time, whether 7 or 14 days checkout length.
- Limited to one (1) Video Game at one time.
The Customer Service Desk can assist with any questions regarding borrowing privileges.
When applying online, please stop by the library within 7 days to pick up and activate your card.
